A progressive and physician friendly organization is seeking an experienced geriatric, internal medicine, or family medicine physician for a highly established PACE program in central Sacramento. Details For this Opportunity:
-100% outpatient medicine with EPIC EMR
-Limited patient volume of highly complex patients 55 yrs. and older
-Probably see 6-10 patients per day
-Provide comprehensive care to vulnerable and comorbid patients of the highest complexity.
– All patients seen in a clinical setting with transportation provided if necessary
– Team of physician and APP cover a patient panel of approximately 150
– Mon-Fri 8am-5pm with call center taking all initial calls
-Salaried position starting at $285,000
-Health/dental/vision at no charge including family
-26 days PTO

-Sacramento Metro location
