VISTA is assisting a long-time client in Connecticut with their search for Nurse Practitioner locums coverage. There is a long-term opportunity for the right provider. This client rarely has locums needs so take advantage of this great chance to join a fantastic team! 
Job Details: 
-Dates Needed: As soon as cred completes – 07/29/2023 with possible extension 
-Specialty: Adult/Geriatric 
-Site: They will be home health visits, generally within a one-hour travel radius/territory
-Reason: Short staffed 
-How Many Needed: 1 
-Certifications: Current Connecticut state NP license 
-Experience: One year 
-License: Connecticut 
-Shifts: Monday – Friday 8:00am – 5:00pm; Participation in On-Call rotation, weekends and week nights (average 1 in every 5 to 9 days) 
-Patients Per Shift: On average 4 to 6 patient homes per day, generally within a one-hour travel radius/territory, they will be scheduled in order of distance to minimize driving 
-Call Required: Yes 
-Patient Population: Predominately geriatric 
-EMR: Ubiquity 
-Call Requirements: weekends and weeknights (average 1 in every 5 to 9 days) 
-Types of Procedures/Cases: Manage chronic illnesses in a complex elderly population with multimorbid conditions focused on wellness, avoiding unnecessary hospitalizations and ER visits, providing patient education about disease processes, improving the overall quality of care, and increasing patient satisfaction in the healthcare system 
-COVID Vaccination Requirement: Yes, no exemptions 
-Training/Orientation: 4 to 8 weeks depending on experience (virtual and in-person)
