We are currently seeking a Palliative Care Advanced Practitioner to join a private practice specializing in meeting the health care needs of adults 65 and older in Norwalk,CA.
Details For this Opportunity:
-Nurse Practitioner-Palliative Care
-Must be very comfortable discussing goals of care and educating on end of life
-Provider will be managing patients in nursing home and at home
-Will be driving within a 10 mile radius of Pasadena
-Full-time, permanent
-M-F, 8 hours day of your choice
-If interested in more work- weekend pay is optional
-See between 10-15 patients/day in 1-2 nursing facilities and home visits
