Minimum Requirements

  • Active Illinois license

  • Board Certified/Board Eligible: Board certified (ANCC or AANP or equivalent approved by IL Board)
  • Certifications: ACLS, ATLS, PALS
  • Licensure: Active, unrestricted Illinois NP license; Illinois Controlled Substance Registration; DEA with Illinois address (Schedules II, IIN, III, IIIN, IV, V)
  • Education: Master's, post-master's, or doctoral degree in Nursing (CCNE or NLNAC accredited)
  • Experience: Clinical experience within past year (new grads accepted; waiver applies)
  • Immunizations: Influenza, Tdap, TB (2-step or QFT), Varicella, Hep B, MMR, Respirator Fit Test
  • Case Logs: Past 24 months
  • CME/Training: 8 hrs Emergency Pediatrics annually, NIH Stroke Scale Certification, Workplace Violence Training, Sexual Assault (SA) Transfer & Treatment, Patient Rights Training, Opioid & Chronic Pain Management, Maternal Hypertension, Maternal Hemorrhage, Antibiotic Stewardship & Attestation
  • Other: Collaborative Agreement required with physician in good standing; Malpractice coverage: $1M / $3M

Physician Assistant (PA)

  • Board Certified/Board Eligible: Board certified (PA-C; must have passed PANCE via NCCPA)
  • Certifications: ACLS, ATLS, PALS
  • Licensure: Active, unrestricted Illinois PA license; Illinois Controlled Substance Registration; DEA with Illinois address (Schedules II, IIN, III, IIIN, IV, V)
  • Education: Graduate of ARC-PA accredited Physician Assistant program
  • Experience: Clinical experience within past year (new grads accepted; waiver applies)
  • Immunizations: Influenza, Tdap, TB (2-step or QFT), Varicella, Hep B, MMR, Respirator Fit Test
  • Case Logs: Past 24 months
