Physician – Cardiology – Advanced Heart Failure and Transplantation in Indiana

Seeking a Cardiologist who specializes in Advanced Heart Failure for a well-established and growing heart failure program supported by one of the region’s largest cardiovascular service lines. This is a unique opportunity for a Heart Failure Cardiologist to step into a leadership-focused role with an existing infrastructure already in place, including dedicated advanced practice support, a robust referral network, and the resources needed to expand advanced heart failure services across the continuum of care. Position located in Munster, Indiana.

• Both inpatient and outpatient responsibilities
• Opportunity to provide physician leadership for an established and growing heart failure program
• Existing heart failure clinic supported by two dedicated nurse practitioners
• Strong built-in referral base from approximately 40 cardiologists and 20 advanced practice providers across four hospitals
• Opportunity to lead the continued development of advanced heart failure services
• Physician-led team model supported by advanced practice providers
• Outpatient heart failure clinic combined with inpatient heart failure consult services
• Outpatient IV diuretic therapy and remote patient monitoring capabilities, including implantable hemodynamic monitoring
• Access to a multidisciplinary care team including cardiac rehabilitation, palliative care, dietitians, and social workers
• Comprehensive diagnostic support including echocardiography, nuclear cardiology imaging, and right heart catheterization
• Collaborative relationship with established Structural Heart and Electrophysiology programs, including TAVR, transcatheter mitral repair, CRT, and ICD therapies
• Leadership support for quality improvement initiatives, guideline-directed medical therapy optimization, and heart failure readmission reduction programs
• No general cardiology call requirements
• Monday through Friday schedule with flexibility
