Minimum Requirements

  • Board Certified/Board Eligible: Board Eligible minimum
  • Certifications: PALS required; additional certifications as required
  • Licensure: Must be licensed in Oregon (OR); IMLC not accepted
  • Credentialing Timeframe: 30-60 days; temporary privileges available
  • Provider Type: Physician
  • Fellowship: Required
